Financial Aid
  • Aid For Part-Time Study (APTS)
    This is a New York State grant for students who are taking 3-11 credits and are in a degree program. Student/Parent must be NYS residents and must complete form with a copy of NYS taxes (most current) to be considered for the grant. Forms must be returned to the Financial Aid office prior to the starting of the semester in question. Student must have TAP eligibility left in order to be eligible. The income cut-off for a dependent student is $50,500 and for an indepent student, $34,500.
  • C Waiver Form
    This form is for students who are in their 4th semester of receiving TAP and failed to meet the overall 2.0 GPA satisfactory progress.
  • Dependent Verification Worksheet
    This form is for students that are required to have parental information on their FAFSA form are selected for income verification. This form is to be completed by the student and parent(s).
  • Drug Conviction Worksheet
    This worksheet is for those students who answered other than "3" to question #23 of the FAFSA. The worksheet is used to determine a student's eligibility for financial aid.
  • FAFSA (Free Application for Federal Student Aid)
    The FAFSA is required for any student who wishes to apply for financial aid. Submitting a FAFSA automatically an application for a Federal Pell grant as well.
  • Independent Verification Worksheet
    This form is for students that are not required to have parental information on their FAFSA form are selected for income verification. This form is to be completed by the student and spouse, if married.
  • Online Access for Aid 2011-2012 New Students
    This form includes instructions for new students viewing and accepting student financial aid award information online.
  • Online Access for Aid 2011-2012 Returning Students
    This form includes instructions for returning students viewing and accepting student financial aid award information online.
  • Selective Service
    This form is for confirming your selective service registration and is for males between the ages of 18-25. All males must be registered with the selective service in order to receive financial aid.
  • Special Condition Form
    This form provides the current financial status a student/family is experiencing. This is for changes of income, family size, one-time payments, capital gains, etc. off your income information for your FAFSA.
  • Student/Parent Assets Form
    This form provides information on the student and parent(s) assets which is needed to determine financial aid eligibility.
  • Summer Aid Request Form
    This form is to be completed by students who will be taking summer school and would like financial aid to cover it.
    Deadline: June 17, 2011

    *Must take at least 6 credits and have your Dean's approval of the classes you wish to take and be a matriculated student (in a degree program).

    The form is now available at the School offices.
  • TAP Change Form
    Form to make corrections such as marital status, adding signatures, exclusion of parental information, and change of address. Complete and submit to the Financial Aid office.
  • TAP Ineligible Waiver
    This is for TAP eligible students who failed to meet satisfactory progress as noted in the College Student Handbook. Students who did not meet these criteria are required to complete this ONE TIME waiver to get their TAP Award reinstated.
  • Untaxed Income Form
    This form is for students/parents who entered zero income on the FAFSA form in addition to the Adjusted Gross Income section(s) of the FAFSA. It provides the Financial Aid Office with a further explanation as to why these sections were answered in the way they were.
