New York State Immunization Law
Effective August 15, 2003 New York State Public Health Law (NYS PHL) 2167 requires all colleges and universities to distribute information about meningococcal disease and vaccination to all students. According to this law, you need to provide either proof of immunization to meningitis, or sign and submit a waiver that states that you understand the risks and choose not to receive the vaccine.
The Meningococcal Disease Information Sheet is available and may be signed electronically. There is also a section that pertains to this on the required Pre-Admission physical form.
