- Pre-Admission Physical Form
- If you are planning on receiving care at the Student Health Center, you must submit a pre-admission physical preferably done by a health care provider who has known you (high school sports physicals are not accepted).
- Meningitis Waiver and Information Sheet
- This is a form with risk information allowing students to elect not to get the meningitis vaccine.
- Release of Information Form
- If you wish to have your medical records transferred to or from another college, you may complete this form and mail or fax it to the Student Health Center.
