The following forms can be completed electronically or downloaded and completed manually.

• Request to Change Major
• Request for Degree in Passing

  Complete and submit to your school office

• Request for Cross Registration

  Complete and submit to your school office

• Request for Lifelong Learning Form (non-matric)

  Complete and submit to the Registrar's Office

• Request for Student Departure/Withdrawal from College

  Complete and obtain the required signatures then submit to the Registrar's Office

• Request for Permanent Home Address Change

  Complete and submit to the Registrar's Office

• Student Drop/Add/Delete Course Form

  Complete and submit to the Registrar's Office

• International Travel Student Release Form

  Complete and return form directly to the faculty member coordinating the international trip.

• Request for Waiver/Substitution of a Required Course

  Complete and obtain the required signatures then submit to the Registrar's Office

• Request Waiver of Policy or Requirement

  Complete and obtain the required signatures then submit to the Registrar's Office
