2018-2019 Forms

4506-T Form (To be filled out by Parents or Independent Students Who Did Not File Taxes)

This form needs to be completed by Students and/or Parents who did not file a tax return in 2016. Be sure to complete questions 1-5, check the box at the right of question 7 for Verification of Non-filing, fill out 9, sign and date. This form will be rejected if it is not filled out completely, and if there are cross outs on it. You can mail it or fax it to your local IRS office located on the 2nd page.

Aid For Part-Time Study

Through the Aid for Part-Time Study program, New York provides tuition to eligible undergraduate students enrolled on a part-time basis in a program of study at a participating institution in NYS. The application, along with the 2016 NYS Income Tax Returns must be submitted to the Financial Aid Office at SUNY Morrisville no later than September 10, 2018 for the Fall 2018 semester.

 

Assets Verification Form (Student/Parent)

This form provides information on the student and parent(s) assets which is needed to determine financial aid eligibility.

 

Consortium Agreement

Students enrolled and matriculated at SUNY Morrisville may receive financial aid for courses taken at another institution if those courses are applicable to their program of study. In order to receive financial aid, federal regulations require that a Consortium Agreement exist before a home institution can process financial aid for students attending another institution (host).

 

C-Waiver Form

This form is for students who are in their 4th semester of receiving TAP (or higher) and failed to meet the overall 2.0 GPA satisfactory progress.

 

Dependent Verification Worksheet

This form is for students that are required to have parental information on their 2018-2019 FAFSA and are selected for income verification. This form is to be completed by the student and the parent(s).

 

Family Educational Rights and Privacy Act (FERPA)

Students may grant SUNY Morrisville permission to release information concerning their student records to a third party by submitting this completed Consent to Release Information Authorization Form.

Identity and Statement of Educational Purpose (To be signed at the Institution)

The student must appear in person at SUNY Morrisville's Financial Aid Office to verify his or her identity by presenting this form and a valid goverment-issued photo identification (ID). This can include, but is not limited to, a driver's license, other state-issued ID, or a passport. The Institution will maintain a copy of the student's photo ID that is annotated with the date it was received and the name of the official at the Institution authorized to collect the student's ID.

 

Identity and Statement of Educational Purpose (To be signed with a Notary)

If the student is unable to appear in person at SUNY Morrisville's Financial Aid Office to verify his or her identity, the student must provide the original notarized Identity and Statement of educational Purpose Form, as well as a copy of the valid government-issued photo identification (ID) that is acknowledged in the notary statement, such as; but not limited to, a driver's license, other state-issued ID, or a passport.

 

Independent Verification Worksheet

This form is for students that are not required to have parental information on their 2018-2019 FAFSA form and are selected for income verification. This form is to be completed by the student and spouse, if married.

 

Non-Tax Filer Verification Form

This needs to be filled out and signed by students who did not file taxes and were not required to file taxes in 2016.

 

Satisfactory Academic Progress - Federal Waiver

This form is used by students who are appealing the loss of their federal financial aid. This form and documentation must be submitted to the Financial Aid Office no later than 30 days after being notified that you are not making Satisfactory Academic Progress (SAP).

 

Selective Service Form

All males who are 18 years of age or older are required to register with Selective Service in order to be eligible for Federal Financial Aid.

 

Special Conditions Form

This form allows for an explanation of changes in financial circumstance that a student or a family may be experiencing (such as adjustments to income, family size, or explanations of one-time payments, capital gaines, etc.) as reported on your FAFSA.

 

Summer Aid Request Form

This form is to be completed by students who will be taking summer courses and would like financial aid to cover them. Students must have their 2018-2019 FAFSA on file and all verification documents submitted to be considered. Deadline: June 15, 2018.

 

TAP Ineligible Waiver

This is for students who are ineligible for their TAP due to failing to meet satisfactory academic progress (SAP) as noted in the College Student Handbook. Students who did not meet these criteria are eligible to complete this ONE TIME waiver to get their TAP reinstated. You must provide an explanation that was beyond your control and provide third party documentation to back your reasoning up. This form and all documentation pertaining to this will be submitted to the Registrar Office for review.

2017-2018 Forms

Aid for Part-Time Study

Through the Aid for Part-Time Study program, New York provides tuition assistance to eligible undergraduate students enrolled on a part-time basis in a program of study at a participating institution in NYS.  The application, along with copies of the most recent NYS Income Tax Returns, must be submitted to the Financial Aid Office at Morrisville State College prior to the start of the semester. Deadline for Spring '18 is February 2, 2018.
